Add / Change Nominee
Digital self-service
- Log in to your insurer portal/app (e.g., LIC e-Services).
- Menu → Service Requests → Update/Change Nominee.
- Add/replace up to 3 nominees and set % share (total 100%).
- Upload ID/address proof if asked → submit and confirm via OTP/e-Sign.
Offline forms (when portal isn’t available)
- LIC: Form 3750 (change), Form 5174 (child policies).
- Private insurers: download “Change of Nominee” form from your insurer’s website and submit at branch/with agent.
Unclaimed proceeds
- Tips: Keep nominee details aligned with your ID proofs; joint policies may require signatures of all policyholders; a nominee is a trustee—final ownership follows your will/succession laws.
